No Premiums Out Here in the Sticks, Only for the City-slickers

If you are in any rural part of PA, you already know this...but has anyone else come up with this?

A carrier has three network levels: partner, select and premium. The smaller networks are in effect the closer that you get to an urban center, Pittsburgh, for example. Out here in the rural zone between Erie and The Burgh, we only have the ability to write Premium network, as fewer people, fewer doctors, more space between the patient and the doctor/hospital/provider and if you need a major service, you have to travel so you have the larger number of providers. You probably get the idea.
