Part B Medications Constant Challenge

I sure would be interested in getting feedback on a better way to deal with injections that are administered in a doctors office and covered by part B. Often the doctor refuses to provide the vial for the medication being injected and sends a prescription to a pharmacy such as Wal Mart where they try to run it through part D and of course it becomes very expensive. Then the long frustrating process starts. Often they don't know how to file a claim for reimbursement through part B. Any thoughts?

I run into this also with insulin for pumps which is also covered under part B.
