Trans Final Expense

Question on Trans final expense. Wrote an 80 year old lady for a small final expense policy, answered no to all the health questions, listed all meds and what they are being taken for. Check the pending app a few days later and they are ordering a PHI, ok, no prob, next thing when I check they are now ordering doc records?!?! Has anyone ever had this before? Its simplified issue (supposto be anyway)
