What is Required to Be on a Direct Mailing in CA ?

I understand an agent license number must be on a direct mailing in California.

Is there anything else ?

I have seen agent name, not affiliated with any government agency, an agent may contact you, insurance related solicitation, information provided by Agent Name LIC #
