Med Sup COPD 71 Year Old

I am not sure if I can help my client::no: a 71 year old male NS to find a medsup ( F or G) . He has COPD NO oxygen, asthma , and triple by pass in 2013 ( no hear attack).
He takes only proair for COPD.

Is there any carrier who would take him? If yes which one and if you tell me premiums for zip code 40475.
