How Does a Mutual Life Insurance Company Work?

I understand that in a mutual, the policyholders are the equivalent of shareholders in a corporation. But what does this mean? They get an annual dividend, but how is this dividend determined? By the number/size/type of the policy/policies they own? Can they determine how the company uses the float? Can they submit proposals to be voted on at the annual meeting?
