UW Help and Recommendation

I have 2 cases that I do not know what to do with below is all the info I have. If anyone can help I would appreciate it. Thanks.

Case 1

Male DOB 12/24/34 Chews Tobacco

Meds

Plavix- less than 1yr prescribed as preventitive didnt have stroke by pass or any other ht condition when prescribed.
Vasotec- preventitive because of diabetes
Prilosec- AR
Actos- Diabetes
Lantus- 10 units a day
Flexeril- Back Spasms.
Norco-Back pain as needed not currently taking
Norvasc - BP
Imdur- BP
Toprol - BP

Case 2

Male DOB 10/09/1955 Chews Tobacco

Meds

Albuterol- Asthma
Gabap
