Just a heads up that Transamerica recently changed the Social Security Benefit Billing Authorization Form and will not accept the Original one from last year.
Here is the nice email they sent me
There are outstanding requirements that prevent us from moving forward with this policy. We need to receive the following requirement(s):
• Fax completed form SSBA 0215 with a new signature from the payor and the current date; we received an older version of this form (SSBA 0714) which cannot be accepted with applications we received on/after 5/1/2015.
