24 Month GI Trial Right

I ran across something on the Medicare website I have never seen before. Where it describes the Medicare supplement guaranteed issue rights, specifically the Medicare Advantage trial right for those who take one at 65, it says that the guaranteed issue period is 12 months, and, "Your rights may last for an extra 12 months under certain circumstances." I have never seen any mention of this additional 12 months. Does anyone know what these "certain circumstances" are?
